To register permanently with the practice you should be resident within the practice area (see map below) and you'll need to complete a registration form and our patient questionnaire.

You can download both forms as editable PDFs, complete them, attach to an email, and send them to us:

You may need to download Adobe Acrobat Reader to edit and save the forms.

At the moment (due to COVID) we will accept forms without a signature.

Please email the completed forms to us at: TAY.monifiethmedicalpractice@nhs.scot. You do not need to attend the practice to register.

Our new patient medical checks are not currently taking place due to COVID, but we will contact you if we have a query.

To register as a patient with Monifieth Medical Practice, you must be resident in our practice boundary area, which is any point within the yellow boundary line below.
